Comments, Guest and Member Information Form

This form is used to collect your data for the Church Office to route to the proper Commission or Task Force. You can use this form to update your contact infomation, request a pastoral visit or a prayer request.

If you are filling out the form electronically be sure to SAVE this form with your first initial and last name for the file name (i.e. SSELC_MemberInfoForm_JDoe) and use the "Email Us" link to send as an attachment to the Church Office. It is recommended to print a copy of your completed form for your records. If you are sending this form by any other means, print your information on a separate sheet of paper and mail it to the address in the footer attention “Comments and Member Form.”
